Concord Hospice, LLC has been delivering quality hospice care to each of our patients. Our team provides and coordinates durable medical equipment, medical and surgical supplies, and a variety of medications related to terminal illnesses.
We give our patients the option to pay from a single source or a combination of sources. These could include private pay, private insurance, Medicare, or Medicaid Hospice Benefit.
Concord Hospice, LLC partners with Medicare and Medicaid programs and accepts their hospice benefits as payments for our services. This is inclusive of all professional visits and services, prescriptions, equipment, and supplies related to the patient’s illness and included in the hospice plan of care.
